herein as "HealthSurveyPanel.com"),

SWEEPSTAKES PERIOD: Entries for the One Year Free Dental Insurance Sweepstakes may be submitted by July 1, 2009 at 11:59:59PM PST and ends Dec 31, 2009 at 11:59:59PM PST (the "Promotion Period"). All entries must be received during the Promotion Period. One (1) winner will be drawn on or about Dec 31, 2009. By participating, you agree to these Official Rules and the Terms of Service. PRIZE: HealthSurveyPanel.com will award one (1) winner with One Year Free Dental Insurance, whose name will be randomly drawn from all entries received in accordance with the SELECTION OF WINNER paragraph of these Official Rules and who is otherwise eligible.
CONDITIONS OF PARTICIPATION: As a condition of participating,
contestants agree that: (i) contestants hereby waive
all rights to claim punitive, incidental, consequential, or any other damages,
other than for actual out of pocket expenses, if any, (ii) all causes of action
arising out of or connected with the entry for a chance to win the One Year Free Dental Insurance Sweepstakes,
or any compensation or credit to be provided, shall be resolved individually,
without resort to any form of class action; and (iii) any and all claims,
judgments, and award shall be limited to actual out of pocket costs incurred,
if any, and in no event shall contestants be entitled to receive attorneys'
fees or other legal costs. All entries for a chance to win the One Year Free Dental Insurance
Sweepstakes shall be governed by New York substantive and procedural law,
without giving effect to any choice of law or conflicts of laws rules or
provisions (whether of the State of New York or any other jurisdiction), that
would cause the application of the laws of any jurisdiction other than the
State of New York. By participating in any and all of the entries for a chance
to win the One Year Free Dental
Insurance Sweepstakes, contestants irrevocably consent that New York
courts shall have personal jurisdiction over any dispute or litigation arising
from or relating to the entry for a chance to win the One Year Free Dental Insurance Sweepstakes and that venue shall
only be in the federal or state courts of New York.
DATA COLLECTION: Information submitted
in connection with any and all entries for a chance to win the One Year Free Dental Insurance
Sweepstakes will be treated in accordance with these Official Rules and HealthSurveyPanel.com’s
Privacy Policy (which may be amended from time to time), provided that in the
event of any conflict between these Official Rules, Terms of Service and
Privacy Policy, the Terms and Conditions of these Official Rules shall prevail.
PRIZE RESTRICTIONS: Acceptance of prize constitutes permission for HealthSurveyPanel.com and its designees to use winners’ name, city and state of residence and/or likeness for purposes of advertising and trade in any and all media now or hereafter known throughout the world in perpetuity, without further compensation, notification or permission, unless prohibited by law. By participating, entrants agree to: (i) be bound by these Official Rules and the decisions of HealthSurveyPanel.com, which are final and binding in all matters; and (ii) release, discharge and hold harmless HealthSurveyPanel.com and each of their respective parents, subsidiaries, affiliates and advertising and promotion agencies, and their respective officers, directors, shareholders, employees, agents, attorneys and representatives (collectively, the "Released Parties") from any and all claims and liability whatsoever for any injuries, losses or damages of any kind resulting from the acceptance, possession or use of prizes (including any injuries resulting from use of the prize). In the event of a dispute as to identity of an entrant, entries will be deemed made by the authorized account holder of the e-mail address from which the entry was submitted and such person will be responsible for complying with all requirements set forth herein. "Authorized account holder" is defined as the natural person who is assigned to an e-mail address by an Internet access provider. Released Parties are not responsible for lost, late, incomplete, inaccurate, stolen, misdirected, undelivered, garbled, illegible or postage-due mail, entries or e-mail; or for lost, interrupted or unavailable network, server, Internet Service Provider, website, or other connections, availability or accessibility or miscommunications or failed computer, satellite, telephone or cable transmissions, lines, or technical failure or jumbled, scrambled, delayed, or misdirected transmissions or computer hardware or software malfunctions, failures or difficulties, or other errors or malfunctions of any kind whether human, mechanical, electronic, computer, network, typographical, printing or otherwise relating to or in connection with any entries for a chance to win the One Year Free Dental Insurance Sweepstakes, including, without limitation, errors or malfunctions which may occur in connection with the administration of the entries for a chance to win the One Year Free Dental Insurance Sweepstakes, the processing of entries, the announcement of the prize or in any HealthSurveyPanel.com-related materials. Released Parties are also not responsible for any incorrect or inaccurate information, whether caused by HealthSurveyPanel.com users, tampering, hacking, or by any equipment or programming associated with or utilized in the entry for a chance to win the One Year Free Dental Insurance Sweepstakes. Released Parties are not responsible for injury or damage to entrants' or to any other person's computer related to or resulting from participating in any entry for a chance to win a Prize or downloading materials from or use of the HealthSurveyPanel.com website. Persons who tamper with or abuse any aspect of the entry for a chance to win the One Year Free Dental Insurance Sweepstakes or the HealthSurveyPanel.com website or who are in violation of these Official Rules, as solely determined by HealthSurveyPanel.com, will be disqualified and their entries void. If, in HealthSurveyPanel.com’s opinion, there is any suspected or actual evidence of tampering with any portion of the entry for a chance to win the One Year Free Dental Insurance Sweepstakes, or if technical difficulties, virus, bugs or other causes compromise the integrity of the contests, HealthSurveyPanel.com reserves the right to modify, suspend or terminate the drawing and, if terminated, at their sole discretion, conduct a random drawing to award the prize using all eligible, non-suspect entries received as of the date of termination. All taxes, fees, and insurance expenses, if any, are the sole responsibility of the winner. No prize substitution, cash substitution, assignment or transfer of the prize is permitted, except that HealthSurveyPanel.com may substitute a prize of equal or greater value in the event it deems necessary. All prizes are awarded "AS IS" and WITHOUT WARRANTY OF ANY KIND, expressed or implied, except as provided by the supplier.
ELIGIBILITY: One Year Free Dental Insurance Sweepstakes is open only to legal residents of the United States and Canada who are 18 years of age or older at the time of entry as confirmed by a valid government-issued photo identification. Employees of HealthSurveyPanel.com, and the immediate families (spouses, parents, children, siblings) and household members of such employees are not eligible.
HOW TO ENTER: NO PURCHASE NECESSARY. All entries must be received through one of our online processes provided through the HealthSurveyPanel.com website or one of its Affiliate sites. By completing the online entry form, you are agreeing to be notified by e-mail if you are the winner of the One Year Free Dental Insurance Sweepstakes. All parts of the entry must be filled out correctly; incomplete, illegible, misdirected, or postage due entries, or entries received through impermissible or illegitimate channels are void. Proof of sending will not be deemed to be proof of receipt by Sponsor. Only one (1) entry per person or e-mail address per process is permitted per day unless otherwise noted. Duplicated daily entries will be voided. Entries become the property of Sponsor and will not be returned. In the unlikely event the Sponsor encounters technical or equipment failures such as telephone network lines failing, computer online systems failing, servers and/or provider services go down, or human error in connection with the sweepstakes, etc., the Sponsor and promoters will not be held responsible. Collection and handling of personally identifiable information will be in accordance to the Privacy Policy as posted on HealthSurveyPanel.com.
If you are entering the sweepstakes via any online method, please remember to set your browser to accept cookies. You may submit no more than one (1) total entry (by any combination of entry methods above). You may not exceed this limit by entering under different e-mail addresses. Entries are not valid unless the entry form is completed properly and a valid e-mail address is provided. Entries are the property of HealthSurveyPanel.com and will not be returned.
SELECTION OF WINNER: One (1) winner will be selected in a random drawing from all eligible and valid entries received. Odds of winning depend upon the number of entries received. Winner will be notified within ten (10) days of the drawing by certified mail, return receipt requested, addressed to the home address provided on winner's entry form. Winner also will be notified via electronic mail, at which time his/her e-mail address will be confirmed. Before receiving any prize and as a condition of receiving such prize, the winner will be required to complete and return an affidavit of eligibility and a liability and publicity release within fourteen (14) days of attempted delivery. If the winner fails to sign and return the required documents within fourteen (14) days of the date on the notification letter, does not accept the prize, or fails to meet any eligibility requirement herein, an alternate winner will be chosen by random drawing to be held as soon as reasonably practicable thereafter. Alternate winner is subject to all eligibility requirements and restrictions of these Official Rules. By accepting the prize, winner agrees that HealthSurveyPanel.com, its affiliates, distributors, advertising and promotional agencies and suppliers and all of their respective officers, directors, employees, representatives and agents shall have no liability for, and shall hold all such parties harmless against damage, loss or injury that may arise in whole or in part, directly or indirectly, from the acceptance, possession, use or misuse of prizes or participation in this Sweepstakes.
HealthSurveyPanel.com will furnish to the Internal Revenue Service any required tax reporting forms relating to the award of the prize including, but not limited to, Form 1099-MISC.
MISCELLANEOUS: This sweepstakes is subject to all applicable federal, state, provincial and local laws. Void where prohibited. HealthSurveyPanel.com shall be entitled to interpret these rules as needed – including, but not limited to, rules regarding entries, selection of winner, deadlines, restrictions on the prize, and eligibility - and all of its decisions are final. By entering, you agree to these conditions. You agree that HealthSurveyPanel.com and any subsidiaries and affiliates, and their consultants, agencies and employees, and any telephone network or service provider shall not be responsible for incorrect, inaccurate or incomplete transcription of entry information; for lost, stolen, late, or misdirected entries; for incorrect e-mail addresses or bounced-back e-mail messages; for destruction or alteration of entries; for unauthorized access to entries; or for any human error, technical malfunction, delay in operation or transmission, lost data, omission, interruption, deletion, defect, communications line failure, computer equipment, software, inability to access any web site or online service, or any other error or malfunction. Proof of sending an entry will not be deemed to be proof of receipt by HealthSurveyPanel.com.
VIOLATIONS OF RULES: HealthSurveyPanel.com reserves the right, at its sole discretion, to disqualify any individual it reasonably suspects or believes to be tampering with the entry process or the award mechanism, whether on-line or by any other means, or to be acting in violation of these rules. A violation of the rules includes any attempt to enter more than once. Any person attempting to defraud or tamper with this Sweepstakes in any way will be ineligible for prizes and may be prosecuted to the full extent of the law.

utilize information obtained from you in order to contact you and/or to offer

Year Free Dental Insurance Sweepstakes. If you would like to have your
name removed from future mailings by HealthSurveyPanel.com, its affiliates,
distributors, advertising and promotional agencies, you may notify

WINNER'S NAME: Winner's name will be posted on the Winners' page at Winners for a minimum of 14 days, after which it may be removed. The Winner's name also may be posted on the other web sites listed above in paragraph 6(b). Additionally, the winner will be contacted via e-mail.
